This is low priority question.
But if anyone wants to look at the .ict files I collected I'll be happy to hear some explanation. Files are here:

I rarely use this TV remote. Almost never. 6131 works great using NEC1 protocol upgrade. But the OEM remote:
1. Messes up Power. Power rarely works. Often requires several presses. It looks, from IRScope, that it doesn't know what protocol to use.
2. More important: Often when I press Vol+ the TV powers OFF. Ouch. Are those two signals soooo close that it sends a wrong bunch of bits?

I'm just curious. Nothing to fix here other than never press Vol+ on this remote when I watch the news in the kitchen.

The POWER learns all show NEC1, dev 4, OBC 8
The VOL+ learns all show NEC1, dev 4, OBC 2

You probably knew that as you have an upgrade that works, so what's wrong with the OEM remote? Hard to say. First thing to always check is the batteries, other than that, not sure what to suggest.

Related question: Considering that Power sends NEC, NEC1 and NEC2 how on earth do you people figure out that it is supposed to be NEC1?
Hmm, Do you not see in IRscope, or in the Summary, those three versions? You just see NEC1?

I suspect that it's that confusion that makes Power almost never work out of the OEM remote.

Batteries are new.

Yeah, I did see all the various permutations, i just guessed that the NEC2 display was caused by two back to back presses, rather than the remote really sending NEC2.

ok so i wasn't going to say anything but curiosity got me.
I recentlly had a similar situation it was vizio but with a jp1 remote
using its built in vizio code. i think vol turned tv on but i cant remember
i work on too many tv's and switch remote codes alot
i will try and duplicate.
But i also wonder if its the tv and not the remote
what is the model of the tv ?
i have also run into OEM remote's that the crystal went bad
dropping the remote several times over the course of years caused a minute
crack in the crystal so freq was off
Has the remote been drpped alot ?

I haven't used built in code. Not even sure if it exists.
Jp1 remotes (6131 and 8910) never do goofy things on this TV. So it's unlikely to be a TV issue.
Remote hasn't been dropped.
Model is VIZIO M190VA-W. Got it in 2011.

When and if i get the time i will do more investigating.
Interesting though is that the vizio i have is also 19 inch just older
VA19L HDTV 10T 2009. I planned on taking it apart someday anyway

Ok so I am going to add more confusion to this topic
Using Potenza ST ADB remote set for vizio built in code 1758
The VA19L HDTV 10T will turn on when I hold the Ch + button
I also have a 37 inch vizio on my bench at the same time
That tv does not turn on with CH +
So ElizabethD problem can somewhat be duplicated
I will have to try other remotes and also take some learns from this remote.
I don't think its triggiring anything for service menu and i plan on
taking the VA19L HDTV 10T apart anyway.
Even though this is a low priority anyone have any other ideas to experiment with ?
